I am assessed with respect to an improvement in the rural municipality.

I hold a permit in the rural municipality with respect to a trailer or mobile home.

I am the spouse of a person who qualifies through registered ownership, assessment or a trailer or mobile-home permit.

I am the chief executive officer of an incorporated co-operative, corporation or religious association assessed on the last revised assessment roll for non-exempt property in the rural municipality.

I declare that the information above is true in all respects, conscientiously believing it to be true and knowing that it has the same force and effect as if made under oath or affirmation and by virtue of the Canada Evidence Act.
